By completing this survey, you agree to the following Terms and Conditions:

    1. Eligibility
      This promotion is open to all participants who successfully complete the survey in full. Participants must provide a valid email address to receive the voucher.

    2. Voucher Offer
      All eligible participants who complete the survey will receive a 15% off voucher code.

    3. Voucher Delivery
      Voucher codes will be displayed on-screen immediately following survey completion.

    4. Voucher Conditions
      The 15% off voucher:
      - is valid for one-time use only
      - can only be used online at www.moodiblends.com
      - cannot be combined with any other offer, discount code, or promotion unless stated otherwise
      - is not transferable and cannot be exchanged for cash

    5. Exclusions
      The voucher may not be valid on certain products or collections, including but not limited to;

      - sale items
      - limited edition products
      (unless otherwise stated by Moodi)

    6. Limitations
      Limit of one voucher per person. Duplicate entries or multiple submissions may be removed at the discretion of Moodi.

    8. Privacy
      Any personal information collected (such as name or email address) will be used only for the purpose of:

      - delivering the voucher code
